The most important safety variable in a filler appointment is not the brand of product. It is how well the person holding the needle knows the blood vessels under your skin. Serious filler complications nearly all share one mechanism: product enters an artery, or presses on one hard enough to close it. Facial arteries connect to the vessels that feed your eye and your skin, so material in the wrong plane can cause tissue death or vision loss. This is uncommon. It is also, when it happens, frequently permanent. Anatomy training is what moves those odds, and asking about it is reasonable, not rude.
How rare, and how serious
The FDA names unintentional injection into a blood vessel as the most concerning risk of soft tissue fillers, and its review of adverse event reports identifies the glabella (the skin between the eyebrows), the nose, the forehead and the area around the eyes as the sites where blockages are reported most often (FDA, Dermal Fillers).
A recent systematic review placed the incidence of vascular occlusion at roughly 0.01 to 0.05 percent of injections, and found the glabella, nose and nasolabial folds most commonly implicated, because those regions sit over vessels that connect directly to the circulation of the eye (systematic review and meta-analysis).
The figures on vision loss deserve to be read slowly. A literature review of filler related visual loss gathered 233 published cases. The nose accounted for about 37 percent of them, the glabella 27 percent and the forehead 20 percent. Hyaluronic acid was involved in roughly half and autologous fat in about 30 percent. Fewer than one in ten of those patients recovered normal visual acuity (review of visual loss after facial filler).
Rare, and usually not reversible. That combination is exactly why this conversation belongs before the appointment rather than after it.
Why anatomy is most of the job
Textbook diagrams show an average face. Real faces vary. The course and the depth of the facial and angular arteries differ between people, and sometimes between the two sides of the same person. An injector cannot see those vessels. They are working from knowledge of which plane is safe at which point, how the tissue layers stack in that specific area, and how slowly and gently to deliver the product.
That is what dissection teaches in a way a weekend certificate cannot. Working on real tissue shows the variation, the layers, and how little distance separates a safe plane from a dangerous one. What does not make an injection safe
This is the part most clinics leave out.
- Aspirating before injecting is not a reliable test. Drawing back on the plunger to check for blood has been taught for years, but the published evidence does not support it as a guarantee. A negative aspiration does not prove the needle tip is outside a vessel.
- Cannulas lower the risk. They do not remove it. Blunt tipped cannulas are recommended for high risk areas, and that recommendation is sound, but occlusions have happened with cannulas too.
- Ultrasound guidance is promising, not standard. Mapping vessels with ultrasound before treating is a sensible adjunct, but it has not been shown to eliminate the complication.
- Dissolvable is not the same as harmless. Hyaluronic acid can be broken down with hyaluronidase, which is a real advantage over permanent fillers, and it is still the material named most often in vision loss reports, partly because it is the most used.
- No credential guarantees that nothing will go wrong. What training changes is how likely a complication is, how fast it is recognised, and how competently it is handled.
Taken together, the honest claim is lower risk and a faster response. It is not zero.
Preparation counts as much as prevention
Ask what happens in the room if something goes wrong. A prepared clinic keeps hyaluronidase on site, knows the dosing and the repeat schedule, and observes you for a period after treating a high risk area instead of sending you straight out of the door. Early signs get checked deliberately rather than hopefully: blanching, mottled discoloration, capillary refill compared on both sides, and pain that is out of proportion to the procedure.
Time is the whole game. Retinal tissue tolerates loss of blood supply for a very short window, and skin ischaemia treated within hours does far better than skin ischaemia treated days later. A clinic you cannot reach outside office hours is not set up for this.
Red flags that need a call the same hour
Contact your injector immediately, or go to an emergency department, if you notice:
- Pain that is severe, escalating, or out of proportion to what you were told to expect, especially once numbing wears off
- A white or pale patch of skin, or dusky, blotchy, lace patterned discoloration
- Any change in vision at all, including blurring, double vision, a shadow across your sight, or pain in the eye
- Drooping, one sided weakness or slurred speech, which is a 911 call rather than a clinic call
- Skin that darkens, blisters or crusts over in the days after treatment
Do not wait until Monday to see whether it settles on its own.
What to ask before you book
- Who is injecting me, what licence do they hold, and what facial anatomy training have they done?
- How often do you treat this specific area?
- Do you keep hyaluronidase on site, and what is your protocol if a vessel is involved?
- How do I reach a clinician at nine on a Saturday evening?
- Is the product FDA approved and supplied in its original packaging? The FDA warns against fillers bought online and against needle free injection devices (FDA consumer update).
An injector who welcomes these questions is already showing you the answer to them.
